2010 HealthPartners® Freedom Plan (Cost) - Wisconsin
HealthPartners has a family of plans for people age 65 and older, or for Medicare-eligible people with disabilities under age 65. These plans are designed to let you live your best life, with support from HealthPartners. The HealthPartners® Wisconsin Freedom Plan gives you the options of medical and prescription drug coverage.
Who's eligible?
To join this plan, you must:
- Be enrolled in Medicare Parts A and/or B;
- Live in the service area
- Not have End Stage Renal Disease (for exceptions, see the Summary of Benefits)
Plan details
Select from one of two medical plans. If you select Plan II, you also have the option of adding prescription drug coverage.

How to enroll
If you'd like to enroll, you can call Medicare Sales and request an enrollment kit be mailed to you. Make sure you've read this important information before applying. Or you can print the HealthPartners® Wisconsin Freedom Enrollment Form
and mail or fax it to HealthPartners.
