2010 HealthPartners® Classic MSHO Plan (HMO)
HealthPartners® Classic Minnesota Senior Health Options (MSHO) Plan (HMO) is designed for seniors on Medical Assistance and Medicare A & B. MSHO offers easy access to primary and specialty health services and your own Care Coordinator to help coordinate your care.
The HealthPartners® Classic MSHO plan is the only MSHO plan in Minnesota that received five stars from the Centers for Medicare and Medicaid Services in 2009.* Information can be found on the Medicare web site at www.medicare.gov.
Who's eligible?
To join this plan, you must:
- Be 65 years or older.
- Have Medical Assistance and Medicare Parts A and B.
- Live in Anoka, Benton, Carver, Chisago, Dakota, Hennepin, Ramsey, Scott, Sherburne, Stearns, Washington or Wright county.

The MSHO plan pairs your medical benefits – including coverage for doctor's visits, inpatient hospital, ambulance and emergency room care, and more – with Medicare Part D prescription drug coverage. But the plan also helps you stay healthy and get the services you need by offering:

Friendly help just for you
When you join our HealthPartners® Classic MSHO Plan (HMO), your personal Care Coordinator will visit you and explain your benefits and how to get the most from the health care system. Your Care Coordinator is a licensed health care professional who can help you improve and/or maintain your health by assisting with prescription drugs and refills; access medical appointments, dental care, transportation, meals, interpreters, and get chore and companion services as well as receive safety modifications to your home as appropriate.
How to enroll
If you select the HealthPartners® Classic MSHO Plan (HMO), enrollment is easy and straightforward. Make sure you've read this important information before applying.
You can call Medicare Sales and request an enrollment kit be mailed to you. Or you can print the HealthPartners® Classic MSHO Enrollment Form
and mail or fax it to HealthPartners.
