2010 HealthPartners® Freedom Plan (Cost) - Minnesota
HealthPartners has a family of plans for people age 65 and older, or for Medicare-eligible people with disabilities under age 65. These plans are designed to let you live your best life, with support from HealthPartners. The HealthPartners® Freedom Plan (Cost) offers you flexibility to choose the combination of medical, prescription drug and dental plans that fit your life.
The Freedom plan offers you:
- Freedom to build a plan that meets your needs and budget. Our affordable plans start at $59 per month and allow you to choose three medical plan, three Part D options and dental options.
- Ability to keep your doctor. Our network has more than 36,000 providers and hospitals throughout Minnesota. You can likely keep your doctor.
- Superior customer service. HealthPartners is the top-rated plan in Minnesota based on customer service and satisfaction, according to U.S. News and World Report.
- Generous travel benefits. You can travel within the U.S. for up to nine months each year, plus have coverage for ER and urgently needed care worldwide!
- Stability. HealthPartners is the nation’s largest consumer-governed, nonprofit healthcare organization and has been serving Minnesotans for more than 50 years.
Who's eligible?
To join this plan, you must:
- Be enrolled in Medicare Parts A and/or B
- Live in the service area
- Not have End Stage Renal Disease (for exceptions, see the Summary of Benefits)

Step 1: Select one of the three Medical Plan options.
- Plan I: most affordable
- Plan II: most balanced
- Plan III: most comprehensive

More than 36,000 providers across the state are part of the Freedom plan's open access network - meaning you can most likely keep your doctor. For more information and to search for your doctor in our network, follow the link on the right.
Step 2: Determine if you want to add Prescription Drug coverage.
If you select Medical Plan II or III, you also have the option of selecting Prescription Drug coverage.
- Standard coverage - with Plan II or III
- Standard Plus coverage - with Plan III only
- Enhanced coverage - with Plan III only

Step 3: Decide if you need the comprehensive dental benefit.
Our comprehensive dental benefit would give you two additional preventive visits annually, plus coverage for most dental procedures. This plan is available with Plans II and III. Find out more here.
How to enroll
Once you've chosen the medical, prescription drug and/or dental plans that are the right fit for you, enrollment is easy and straightforward. Make sure you've read this important information before applying.

and mail or fax it to HealthPartners.
